- Fields marked with asterisk (*) must be completed.
- Give full and accurate information when completing the form as failure to do so could jeopardise your chances of admission.
- If you are offered provisional admission, you will be required to produce the original copies of all your certificates at the point of registration.
- The Polytechnic is not bound to give reasons for its rejection of any application and will not enter into any correspondance on the subject. The decision of the Institute is final and applications are accepted on conditions.
- Ensure to print at least 2 copies of your application form and keep both your PIN card and bank teller for future reference.
